Please be sure to bring the following to your tax appointment:

1. Proof of identification

2. Social Security cards for you, your spouse and dependents, and/or a Social Security Number verification letter issued by the Social Security. Or bring an Individual Tax Payer Identification Number (ITIN) assignment letter for you, your spouse, and dependents, if you have been assigned an ITIN. Bring these even if you have done your taxes with VITA in the past.
3. Birth dates for you, your spouse and dependents on the tax return
4. All wage and earning statements (Form W-2, W-2G, 1099R) from all employers
5. Interest and dividend statement(s) from your bank(s) (Forms 1099-INT, 1099-DIV), if any
6. Bank routing and account numbers to set up Direct Deposit, if you are wanting Direct deposit
7. Total paid for daycare provider and the daycare provider’s employer information (the provider’s Social Security Number or the provider’s business Employer Identification Number (EIN))

8. Any other tax documents to be used in the preparation of your return

9. If you and your spouse are filing a joint return, you both must be present during the appointment.
